‘Meds-to-beds’ programs aim to prevent re-hospitalization

Hackensack University Medical Center is among some hospitals nationwide that implemented a “meds to beds” or “meds in hand” program, in which prescription drugs are given directly to patients just before they are sent home. Two studies have indicated the program can reduce readmission rates. In 2011, Louis Teichholz, M.D., chief of cardiology, in collaboration with the medical center’s pharmacy services department, launched the meds-to-beds program that targeted recent stent patients.
