If you are hospitalized, our geriatric medicine specialists will collaborate with specialty physicians— including cardiology, orthopedics, surgery and psychiatry — to ensure you receive the highest quality care. We also offer emergency and trauma consultations.
Delirium (confusion) prevention and management
As part of our efforts to address delirium in hospitalized patients, Hackensack University Medical Center implemented the Hospital Elder Life Program (HELP) in 2008. HELP is a comprehensive, evidence-based program that provides assessment and intervention for older adult patients to prevent and manage delirium should it occur during their hospitalization. Using HELP model, HackensackUMC created first disease specific certification in geriatric delirium in 2011.
This is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with the American Hospital Association and the Catholic Health Association of the United States. It’s a seven month learning community to make your health system age friendly. We are targeting Mentation, Mobility and Medications for this initiative.
